"We", "Our", "Us") and the information we collect. Even if you never purchase any products or services
offered on the YouDecide platform, we may still have some information about you. Information may have
been provided to us from your employer or member group that identifies you as an eligible participant. This

YouDecide never sells the personal information of its platform participants, eligible members, or clients.
Because of this, we do not have a Do Not Sell My Personal Information link on our platform.
Most users gain access to the YouDecide platform through their employer or member group. In these
cases, YouDecide has an agreement with your employer or member group (our client) to offer you
products and services via our platform on their behalf. YouDecide is obligated to process the personal
information we are provided by our client (eligibility information) in accordance with our client agreement.
In such circumstances, the employer/group is ultimately responsible for determining how your personal
information will be handled as well as which providers appear on the platform. Thus, you may need to
contact your employer or member group rather than YouDecide to exercise the right to:
request an accounting of disclosures;
correct any incorrect information;
request deletion of your information;
complain if you believe your privacy rights have been violated;
terminate your participation on the platform;
If you are not certain whether we are acting on behalf of your employer/group in your situation, please contact YouDecide.
Be aware that even if you do not register or purchase anything on the YouDecide platform, YouDecide
retains your eligibility information for as long as you are in the eligible group. Your eligibility information
may contain personal identifying information. To omit your eligibility information from being given to
YouDecide, please contact your employer/group.
Keep your information and preferences updated on the YouDecide platform and with your
employer/group and the provider partners with whom you have interacted.

If you suspect YouDecide is the cause of a data breach or inappropriate disclosure, or if you feel
your account has been compromised for any reason, contact YouDecide immediately.
YouDecide does not receive financial incentives for collecting your information. YouDecide receives a
commission for the sale of insurance policies, just as any insurance agent would. YouDecide may receive
fees from merchants to be placed on our platform and a fee for featuring a provider in our marketing
Employers, member groups, and providers on our platform may engage intermediaries to assist in their
platform activities. For example, if your employer uses a payroll processing company and offers payment
for insurance premiums via payroll deduction, your employer may direct YouDecide to interact with that
payroll processing company on their behalf. In the descriptions below of information exchanges with your
employer/group or with a provider, the terms employer/group and provider are inclusive of any
intermediaries they may direct us to interact with on their behalf. If you have questions about who these
intermediaries are, please contact your employer/group or YouDecide.
From your employer/group: The type of information each employer/group sends varies. It may be as little as only your name, work email address, and employee ID number, or include other elements such as your home address, date of birth, and so on. Often, the data elements your employer/group sends us are related to the voluntary benefits, discounts, and features they want us to offer on the platform. To find out the exact information your employer/group sends us, contact your employer/group.
From providers: We may collect information from our providers regarding any product/service in which you enroll.
While interacting with the platform, you may provide us with information. Here are some examples
Registration: When you register on our platform, you provide your name, a password, email
address, and set your preferences for cookies and marketing communications. Within the User
Profile section of the site, you may optionally enter a phone number and home address if you
choose but this is not required.
Actions/requests you initiate and our fulfillment of those:
We collect information from you in the course of providing a quote for services. For example, if
you request an auto or home insurance quote, we may collect information about your vehicle(s)
and drivers, or about your home. We share this information with applicable provider partners on
our platform so they can provide the quote and we can deliver the quote back to you.
If you enroll in some types of voluntary benefits, we may collect additional information
about you and/or the dependents you wish to cover. This varies by the provider, policy, and plan type. For example,
when you enroll in vision insurance, you may need to provide information on other family
members that you wish to be covered.
If you contact YouDecide advisors, the advisor makes notes about the call
and the conversation you have with them may be recorded for quality and training purposes.
If you purchase services on our platform that are eligible to be paid for via payroll deduction, then on your behalf, we obtain billing information from the provider partner to facilitate payment processing. This information may include a policy number, the premium amount to be collected, and other information such as dates and transaction type, for example. Based on your policies and payroll deduction amounts, YouDecide coordinates with your employer and insurance carriers to reconcile the payments.
Other information you may provide: You may provide us with other information either online or by
phone. This information includes forms you may fill out, surveys, nominating a provider, and
messages or correspondence you send us. We use this information to help provide you with goods
and services on our platform and to resolve any issues you encounter while using the platform.
Cookies and other tracking technologies may be used by YouDecide to analyze trends, administer the
website, track users' movements around the website, and gather information about our user base as a
whole. We gather some information automatically and store it in log files. This information may include
internet protocol (IP) addresses, browser type, internet service provider (ISP), referring/exit pages,
operating system, date/time stamp, and/or other data to help provide usage statistics.
Here is how we use the cookies and elements we store about your platform session:
Strictly Necessary Cookies: These cookies help to ensure network security (i.e., session encoding),
do load balancing on servers, and help us understand how to interact with your equipment
(browser and operating system) so that our platform can display correctly to you.
Analytic Cookies: These are used to gather site usage statistics, such as to determine which
offerings are our most popular, and to help fine-tune our network's performance. We use Google
Analytics to assist with these analytics and provide reports.
Functional Cookies: These cookies enable the chat feature on our site and remember your
preferences from one visit to the next. If you do not allow these cookies, you will not have the chat
functionality and may have to continually see messages you may have already acknowledged and
no longer wish to see.
Usage statistics are aggregated (non-personally identifiable) and are provided in reports to our clients and
provider partners. We do NOT use cookie and tracking information to provide you with targeted ads or to
do behavioral profiling for marketing purposes. You can control the use of optional cookies via the
Manage Cookies option on the platform. If you choose to disable all cookies from within your browser, the
YouDecide platform will not work. If you set your browser to Do Not Track, this will not affect the use and
function of the YouDecide platform, as YouDecide does not respond to Do Not Track signals.
Our provider partners may employ their own cookies and/or tracking technologies. If you click on a link to a
provider partner's website, you become subject to that provider's cookie policies. Refer to the provider's
YouDecide does not sell your information and does not share your information with any external entities
who are not involved in providing services on our platform. We have agreements with our platform
providers that oblige them not to sell or share your information outside of the terms of that agreement, and
to provide safeguards to ensure your data is protected from unauthorized disclosure.

YouDecide is legally required to share your information under these circumstances without your

Each provider partner participating on our platform collects information based on the services or products
they offer and according to their operating policies and underwriting guidelines. In some cases, such as
when providing a quote for services, YouDecide may collect information from you on their behalf, securely
transmit the information to the provider partner electronically, then transmit information back to you. In
other cases, we provide a link to the provider's website so that you can complete the quoting or sign-up
process there (external to YouDecide).
Our provider partners will not use information collected by YouDecide to telemarket their services to you,
nor will they place you on a mailing list. If you interact with or purchase products or services from a
provider partner, you become part of their customer base and they will handle your information and
communications in accordance with their company's operational and privacy policies. This is outside of
YouDecide's control. We encourage you to obtain and read the privacy policies of each of the provider
partners with whom you do business.
YouDecide enlists the services of third-party business partners to assist us in carrying out our mission.
Among these are cloud hosting, data analytics, marketing services, VoIP services, and data security
consultants. We have signed agreements with each of these entities and they are required to maintain
confidentiality and security of the data they handle. Each is also required to implement physical, technical,
and administrative safeguards to protect any personally identifiable data and to alert YouDecide of any
security incidents or data breaches involving your data.
The California Consumer Privacy Act (CCPA) and other state regulations may require us to disclose:
the categories of personal information we have collected in the last 12 months;
the sources from which the information was collected,
the business purpose for which the information was collected, and
the third parties with whom we shared/disclosed that information in the past 12 months.
This information can be found in the charts below. Depending on your employer/group's implementation
and your use of the YouDecide platform, not all categories of information may apply to you, and not all
examples of each category may be collected.

You provide personal identifiers and/or personal information during the course of
using the platform, obtaining a quote, purchasing products/services. Audio from your
calls to the call center are recorded for quality & training purposes.
Your employer/group may send us an eligibility file containing personal identifiers
and personal information. Your employer may send us payroll information for linking
up the payment of premiums via payroll deduction (if applicable).
Providers send us product/service pricing needed for payroll processing information
when you enroll or purchase insurance or other products/services via payroll
deduction (if applicable)

Entities We Share Your Personal Information With
Our data services
YouDecide uses third-parties for the distribution of our marketing newsletter, compilation of platform activity for reporting, and a VoIP service. Some personal identifiers and platform activity may be shared with these entities.
Some platform providers require YouDecide to share your eligibility information (including
some personal identifiers) with them so that they may provide services to you. If you
request an insurance quote or purchase an insurance product/service, YouDecide will
share your needed personal identifiers and personal information with them. If applicable,
YouDecide shares payment information for insurance premiums with the respective
provider (to be used in payroll processing).
YouDecide shares payroll deduction information with your employer for products/services you purchased (if applicable).
Additionally, we may use the information we collect to comply with regulations and other legal obligations,
to detect and prevent suspected fraud, illegal activities, or threats to our network.
This section explains how you can exercise your data rights. YouDecide will not discriminate or retaliate
against you for exercising your rights. Many of these are the rights granted to California residents under
the CCPA. The particular rights in your state may vary from this slightly, and if so, we'll tell you.
If you believe some information we have about you is incorrect, please contact YouDecide. Some
information we have about you may have been provided by your employer, member group, and/or a
provider partner. In that case, we will direct you to the appropriate source who can assist you with
If the information is within our control to correct, we will respond to you and/or make the change
effective within 15 days. This service is provided free of charge to you
Your ability to obtain this information free of charge may be limited to twice in any 12-month period
or have other limitations depending on your state of residence. YouDecide's ability to provide this
information to you may be limited to only the 12-month period prior to the date of your request or
may be limited by the data's retention period.
To request a copy of the information we have about you, contact YouDecide or contact your
employer/group. We will respond to your request within 45 days of its receipt. If we cannot respond
within that time, we will inform you of the need for an extension within that time.
In our reply, we will list the data elements we have on you, the organizations and/or individuals we
shared your information with, and why. We will include disclosures you asked us to make, such as
when we communicated with an authorized person on your behalf or sent your information to a
provider partner upon your request. We will include your current opt-in/out status.
The entities we share information with are dependent on your employer's/member group's implementation of the platform. With some implementations, we may share your eligibility information with some of our provider partners for example, those who pre-qualify you for group rates. If your platform implementation does this type of sharing, you will need to contact your employer/member group and request that they no longer share your information with YouDecide. This would make you ineligible to take advantage of the savings and programs offered through your YouDecide program.
With some platform implementations, the employer/member group does not share eligibility
information with YouDecide. In these cases, your information is only shared with providers with
your consent. For example, if you request an insurance quote, the information you enter is shared
with providers who will be responding to the quote request. If you choose not to share your
information, you can abort the quote and no information will be shared.
If you gained access to YouDecide through your employer or member group, you must contact
your employer/group. Your employer/group will remove you from the list of eligible participants and
will let YouDecide know. This also happens automatically when you leave the employment of your
employer or cease to be a member of an eligible group.
If you did not receive access to YouDecide through an employer or group, or your employer/group
does not send YouDecide eligibility information, you may contact YouDecide and request to have your account deleted.
Whether you request that your account be deleted or you become ineligible (usually due to leaving
your employer/group), the effects are the same. Your login will be deactivated and you will no
longer be able to use the YouDecide platform.
If you were enrolled in payroll deduction as the means for payment, you may or may not be able to continue the payment on a direct bill basis, depending on the provider and the product(s) involved. YouDecide will notify the affected provider(s) and they will contact you. Alternatively, you can reach out to the provider yourself. Be aware that in some cases, deleting your account or loss of eligibility may mean that a product or service is no longer available to you or that the pricing may change.
YouDecide will cease processing/sharing of your data. After 60 days, your platform activity data will
be removed or de-identified so that it can no longer be traced back to you. We may be
contractually, legally, or financially required to keep some of your billing information, enrollments in
insurance policies, and payroll deduction amounts for a period of time beyond the initial 60-day
If you want to re-enable participation in the platform or if you become eligible again through another
means (another employer/group), you will need to start a new account. Your former account
information will not be retrievable.
You can opt-in or opt-out of receiving marketing communications from YouDecide. These marketing communications contain special offerings from our provider partners. The opt-out (or opt-in) will take effect within 10 days of your designation. You can control your subscription preferences using any of the following methods:
While logged into the platform, navigate to My Account and click on Account Settings. There will
be a checkbox where you indicate your marketing preferences. You can set this on or off as you
like at any time.
When/if you receive a marketing email, click on the Unsubscribe link contained within it.
If you have given another person power of attorney or if you have a legal guardian, that person can
exercise your privacy rights and make choices about your information. We will need to make sure
that person has this authority to act for you before we take any action. The authorized person may
contact the YouDecide Privacy Officer via one of the contact methods.
Unless we have this authorized person's information on file, YouDecide cannot share your
personal information with them. We take your privacy seriously and will not speak to family or
friends who contact YouDecide unless they have been designated as an authorized person on your

If we were unable to resolve your issue, and you received access to the YouDecide platform
through your employer/group, contact your employer/group.

We value your privacy and guard your personal information as if it were our own. We have an ethical
responsibility to protect it, not just a legal responsibility. We protect information provided to us via the
YouDecide website in the same way we protect information provided to us over the phone or any other
way (e.g., fax, email, etc.).
YouDecide uses state-of-the-art tools and methods, including encryption, to protect your information at all
times - when collected, stored, transmitted, or displayed. While no security mechanism can be 100%
guaranteed, we provide a secure platform utilizing SSL protocols that encrypts your data over the Internet.
All transmissions containing personal information such as to/from your employer/group and to/from our
provider partners are encrypted. Within YouDecide's network, firewalls and authentication rules limit
access to your personal information to only authorized YouDecide workers. Our staff is trained on privacy
and security safeguards and understands the consequences if these are violated. All system interactions
are monitored, logged, and reviewed, and we have automated alerts to warn us of unusual activity.
Independent security firms perform penetration tests to verify our safeguards are working appropriately to
protect our network and your data. These are just a few of the many examples of the security measures we
YouDecide strives to make its platforms compliant with current website accessibility standards, including
the Web Content Accessibility Guidelines (WCAG) 2.1 and Section 508, in accordance with the Americans
with Disabilities Act. If you encounter difficulty using our platform, please contact YouDecide to let us know.
All of YouDecide's data is stored and processed within the confines of the United States of America (USA).
If you access your YouDecide account from outside the USA, be aware that the country in which you are
located may have different data protection laws. If you are in the EU or UK, YouDecide relies on the
safeguards set forth in Articles 46 and 49 of the GDPR to transfer your data across international borders.
YouDecide does not target or market to audiences in the EU or UK.
We retain your data for as long as it is needed for processing your requests, complying with our client and
provider agreements, and for our own internal legal or financial requirements. We will retain your data as
long as you remain eligible to help answer questions. However, the following are examples of data that are
deleted or de-identified sooner.
Recordings of call center (customer support) interactions are deleted after 12 months.
Eligibility and payroll processing files exchanged with provider partners and/or clients
(employer/member groups) are deleted after 12 months.
Enrollments in voluntary benefits and remittance files that were exchanged with employers are
deleted after 24 months.
Information you entered to obtain a home or auto insurance quote is deleted after 12 months. Note
that provider partners who received the quote information may have different retention periods.
Within 60-days of a client's termination, the member activity data of all members of that group is
removed or de-identified, as well as all file transmissions between YouDecide and the
employer/member group. Marketing data, analytics data, and emails that may contain personal
information are also deleted or de-identified.
We are legally obligated to keep information pertaining to insurance enrollments and/or payroll
deductions for up to 7 years after the last billing or enrollment date, whichever is later. After that
time, the data will be deleted or de-identified.
We notify our provider partners to delete/de-identify the data of all members of a terminated client
group. However, the provider partner may be obligated to retain your information according to their
own retention criteria if you had previously engaged directly with them.
If we make substantive changes to our privacy practices, we are required to notify you. Notification will be
made via the platform and via email. The changes will apply to all information we have about you. Check
the effective date at the top of this notice to verify you are reading the most current version.
If YouDecide is involved in a merger, acquisition, or sale of all or a portion of its assets, you will be notified
of any change in ownership, uses of your personal information, and choices you may have regarding your
